To Parish Staff:

  • Always print a new supply of the Petition Forms from our website. By doing so, the Petition Form is always current, reflecting any possible new changes.
  • If you have any questions or difficulties, email the Metropolitan Tribunal or call 713.807.9286.

Parishes are expected to give to every Petitioner the following 3 parts:

  • Letter of Introduction (pages i-iii)
  • FAQS (pages I-XII)
  • Formal Petition form (pages 1-23)

Parishes are encouraged to print as follows:



- pp. 1-14 on white paper, one sided only.
 

- pp. 15-23 on blue paper, front and back.
