Liturgy Preparation

The Liturgy Preparation Form is to be completed and submitted no later than six weeks (45 days) prior to the scheduled liturgy.

Notes for Submitting Liturgy Request & Liturgy Preparation Forms

Our preference for submitting this form is via email. The form can be submitted via email by the following process:
1. Click on the form in the preferred language and Save it to your workstation
2. Open the MS Word document and fill out fields which allow for user input
3. Re-save the file under a different name
4. Fill out the form entirely. Indicate all prayers, e.g. Confirmation A, or 4th Sunday of Ordinary Time (OT)
5. Indicate the readings which will be used.
6. List all musical elements by title, composer and hymnal.
7. Attach the completed document file to a new email message and send to worshipoffice@archgh.org as indicated on the form.
