Application via postal mail consists of the
following process:
Download the
PDF version of the grant
application document and save it on
your local workstation.
Print out the PDF version of the
grant application document and fill
in all fields. You will need a
total of three copies of your
completed grant application, two to
be mailed in and one for your
records.
Postal mail TWO COPIES of the
completed grant application document
to the CCHD Office at the
address below.
